NEW CUSTOMER INFORMATION
Our office is located at 401 South Meridian St. in Lebanon Indiana. Our office hours are 7:30 a.m. to 4:30 p.m. Monday through Friday. We only do new service applications from 7:30 a.m. to 4:00 p.m..
To establish a new account for service, you must complete a New Service Application(Commercial or Residential) and provide a picture ID. Lebanon Utilities may verify your identity through a credit reporting agency and a deposit may be required to establish service. Deposits are based on your credit history. If you are renting or leasing you must provide a lease or a utility transfer form signed by the landlord. We must have a picture ID for everyone on the lease.
