NEW CUSTOMER INFORMATION
Our office is located at 401 South Meridian St. in Lebanon Indiana. Our office hours are 7:30 a.m. to 4:30 p.m. Monday through Friday. We only do new service applications from 7:30 a.m. to 4:00 p.m..
To establish a new account for service, you must complete a New Service Application(Commercial or Residential) and provide a picture ID. Lebanon Utilities may verify your identity through a credit reporting agency and a deposit may be required to establish service. Deposits are based on your credit history. If you are renting or leasing you must provide the Verification of Utility Customer form. We must have a picture ID for everyone on the Verification of Utility Customer form.
Please give us two working days for the new account to be set up.
